A patient experience connected to existing care systems

Patient portal development gives patients a defined way to book care, view authorized information and communicate with their care team. Pharos builds the web experience and its application workflows around the systems your organization already uses.

In CareConnect, we developed appointment tools, access to medical records, virtual visits and secure messaging. The published case describes an API-first layer above existing clinical systems and user research with patients and front-desk staff. That delivery model supports a portal scope without requiring a complete EHR replacement.

Configure an existing portal or build a custom experience?

An available clinical-vendor portal is a useful starting point. Custom work needs a specific patient or staff workflow that the existing product cannot support.

A sample appointment journey for portal acceptance

Illustrative checklist, not a CareConnect artifact. Walk through booking, rescheduling and cancellation with approved test accounts. Validate both the patient view and the clinical-system state.

  1. A slot changes before confirmation

    Select an appointment that becomes unavailable before the patient confirms it. The portal should show the agreed recovery path and distinguish a request from a confirmed booking. Check the system responsible for slot ownership.

  2. A patient reschedules

    Move a confirmed visit to another time. Verify the previous booking state, the new confirmation and the staff view. Decide which notifications follow the change and how their delivery failures become visible.

  3. Access changes between users

    Check patient access, staff access and any separately scoped caregiver role. Attempt to view a record outside each account permission. Confirm that account recovery does not bypass the authorization boundary.

  4. The clinical connection fails

    Interrupt record retrieval or appointment exchange. Review the message shown to the patient and the issue visible to support staff. When the connection recovers, reconcile pending operations without silently confirming an unaccepted request.

Patient and staff workflows to scope

  • Identity and authorized records
    Connect patient authentication to the records each account may view. Define recovery and session behavior, record-release timing and the rules for an optional caregiver role. A portal should expose the permission boundary clearly without asking patients to understand backend identifiers.
  • Appointments and reminders
    Build booking, rescheduling and cancellation around the clinical system that owns availability. Show confirmation and pending states separately. Reminder channels and delivery tracking are selected with the organization so staff know which exceptions still require a manual follow-up.
  • Care-team communication
    Route messages to an agreed team or queue and display the operating expectations patients need. Virtual visits can be added where the care model supports them. Each module needs its own staff handoff and failure behavior; a communication screen alone does not define clinical operations.
  • Integration and controlled rollout
    Connect the approved interfaces and pilot the selected journeys before expanding access. EHR integration services covers mappings and exchange behavior beneath the portal. Keep the patient experience and the clinical source of truth aligned when an operation fails or a record changes.

Questions before building a patient portal

  • Can caregivers or guardians use the portal?

    Delegated access is a separate workflow to define with your organization. Specify which records and actions a delegate may access, how authority is established and what changes when that authority ends.

    It should not reuse unrestricted patient credentials.

  • Will patients need a native mobile app?

    A responsive web portal can support the agreed journey without a separate app release. A native app becomes a scope decision when device features, distribution requirements or the patient usage model justify it.

    We assess that need before adding another platform to maintain.

  • Can secure messaging replace telephone support?

    Messaging needs a staffed operational model: who receives each request, how patients understand response expectations and where urgent concerns are directed. The portal can expose those rules, but it does not create the care-team capacity to answer messages.

  • How do you evaluate usability before launch?

    Use a pilot with representative patients and staff to observe completion of the selected journeys and support handoffs. Include accessibility requirements and language needs in the acceptance plan.

    Compare the observed results with the organization's starting process rather than assuming adoption.
